EDIT: on the original topic, the best alleycat I've raced here was the Halloween one, which was a Misfits theme. Last checkpoint was singing an entire Misfits song with a live band in somebody's basement while wearing a devilock.
I kind of think any checkpoint in which something is drawn or written on somebody's face with permanent marker is always good. I had to go to work and wait tables at brunch the next day after the Misfits alleycat with a mustache and goatee drawn on my face.

In one of my races people had to remove and replace a tube from a wheel off a kids bike. Not so amazingly, for the ATL scene at least, probably half had a lot of trouble doing it. The people working the checkpoint had to help a few riders after they struggled for several minutes.
I'm always a fan of making people ride bad roads and back alleys. Make it something that is ridable only at slow speed on a road bike. Short enough that if people run they can make about the same time as someone who rides. People on mtb's would obviously have an advantage.
One race had you write a father's day card and address it to your dad.
Get a can of beer at one of the first checkpoints. Chug it at one of the last when its all shaken up and warm.
I had an idea for a race where at each checkpoint you get a small bag of sand. The earlier you get there, the heavier the bag. Fast riders could end up carrying an extra 25lb by the end of the race.

How about making straight edge kids eat a couple table spoons of wasabi? That **** is rough.
I heard at some races they get punched in the stomach. At the check point that is.

Something with legos. Buy a bunch legos in bulk somewhere. Give each person the same piece or pieces at each checkpoint. Have people build the finished piece (something standardized, like a bicycle) at the next to last checkpoint. Have them superglue them at the finish. Make sure not to have nail polish remover there so at least one person would have to go home with legos stuck to their fingers.
I really like the sand idea. It would be necessary to finish on a hill though.
