You could see if someone with a KHS Flite 100 or Mercier Kilo TT is selling their stock fork. It fits all your criteria.

Surly cross-check has everything you require, but has canti studs. Unless you switch to a threaded headset I don't know of any other options.

If you can live without eyelets there's the soma and surly steamroller as well.

I can't deal with the studs.
I can go to a threaded fork if I really need to, but I already have the parts and stems I need to use a threadless, so I was trying to save money on stuff I don't need.
My headtube is 200mm long, btw.
