I have.
It was on my 2004 fuji track(SS). Im 5'11" and weigh about 210lbs.
It happend when i was riding real fast and hit a small bump. My first thoughts were that i tripped the bearings or something. When i took it to my lbs the menchanic opened it up and showed me the axle broken in half. He said he never seen that before.

Bikes: '92 novara ponderosa, '74 schwinn le tour, Novara fusion, novara transfer, novara randonee(2), novara careema pro, novara bonita(2).

I've only ever heard of it happening on freestyle mountain bikes a few years ago before extra burly freestyle parts were common, never on anything else, ever.
It's defective, get a free replacement no matter what the age.

Don't get to thinking you are Superman. The axle obviously had a material flaw and was defective from the start.

In the twenty or so years I spent in the wholesale and retail cycling industry, bottom bracket axle breakage was extremely rare, almost non-existent. All breakage cases were due to defective castings or forgings.
