I've had two wrecks in the last three years:
1. On the South Bay bike path at night when I didn't have enough light, and went into the sand on a curve. Unfortunately I landed on the pavement though - OUCH! I now have a much better light.
2. Coming home late at night, I cut through a back alley and wiped out on some torn up asphalt. I was on the ground before I knew what happened.
In both cases, I was pretty tired. I'm usually much more careful.

2006 was horrible, six crashes two of which put me in the ER.
2007 was wonderful, kept the bikes upright (or at least the rubber side down) the whole year.
2008 I am aiming to replay 2007, at least as regards to crashes.

One broken pelvic bone in September of 06 after crossing wheels with my buddy. Landed on my head last year some time; I was watching a tall female runner instead of the trail and ran right off of it. I stayed on until I went to get back on the trail and misjudged the lip coming back on and also I didn't see the succulents that lined the trail there.

Twice in 2006, once in 2007, and ten days ago. Went down hard on the Ballona Creek path, fortunately nothing was broken on either me or the bike, though we both were knocked around pretty good. I got the stitches removed from my elbow today, ribs and hip are still pretty sore.

Crashed last year and got my face and arms all banged up. I looked like a battered wife for a couple of days after the crash. That was gross.

Yes.
Hah! I was on the Goldline last night and I started talking to this lawyer (middle aged) and he told me
"I used to ride a bike to work but I kept getting hit by cars. I was in the hospital every six months"
Not exactly what I'd like to hear from someone when I'm on my way to a meeting about how to get more people to use bikes in LA...
He'd been mostly right-hooked.
This is an excellent page...
