It sounds like he stopped, checked for approaching traffic in the road, looked at the corner for pedestrian traffic on the sidewalk, and started to make his turn. That's when she hit him on the right side. She probably went head first into the side of his truck.
I've had sidewalk cyclists come out of nowhere before after I'd checked all around. That's why riding on the sidewalk is so dangerous.
