Ouch on the Ergon grip surgery! Trekking bars (with a longer stem) might be another option. I have one set with Gripshifts and another with Ergon GP1s + Alfine trigger shifter. I'm sure one of Ergon's Rohloff/Nexus GP1 short (96.5 mm) grips would work fine.
