The fork crown looks to be
a Cinelli CCA fork crown. Probably not too uncommon. I only recognized it because I think it's a match to what I have on my Look.

I have no idea what your experience was with Mrs. Mikkelson, but I spoke with her on the phone a couple times. Inquired about replacing a steerer tube (which I never did) and she was quite pleasant and helpful enough.
