Looks like it. Actually, look at the fork crown race and bottom headset cup. They should be more flush with each other. Maybe the steerer tube is bent at the base. Also, looks like the seatpost was too small and the binder got mashed together. Poor frame.
How do you see that it's bent or not flush? I can take a closer picture if you don't believe me
