Sidi 6.6 mega. That shoe has the adjustable heel retention device which allows you to dial in your heel fit. The mega is a wide shoe. They also make a "SP" verson,,,Speedplay sole. No longer do you need that extra shim.

Not cheap. But they do meet all your needs if they are wide enough. Mega is roughly EE/EEE.

Problem with people with foot shaping issue is they never quite work out that to get a good shoe is going to cost you a few bucks.

Also, wherever you get these ask if the fitter does cleat setups. Your symptoms also sound as if you do not have your cleats positions properly.

Tips: try shoes on at the end of the day when your feet are going to be more swollen. Know that if you go with this shoe, it is not leather but a synthetic. I'll stand in mine for five or 10 minutes, then retighten them when the synthetic material warms up some. Also, make sure you stand in the shoe to check fit.

Try shoes that are a big bigger and a bit smaller just for checking fit. You can have a wide foot with a low arch and that will impact fit as well...in addition to width and length.
