have you really carefully examined where you are getting the flat? look at the hole in the tube: is it where the tube sits on the rim or where the tube interfaces the tire? that may help you narrow down how/where the flat is coming from. make sure you don't have any sharp burrs on the inside of the rim or that no spokes are exposed on the back of the rim. maybe also try a better rim strip (are you using velox?).
