This is borderline political, but in the interest of maybe saving someone's sight, I hope I'm excused. I believe Indiana is one of those states that refused the Medicaid expansion under the ACA. If the OP would simply high-tail it to a nearby state that didn't refuse to expand its Medicaid program to low-income adults. he may find that the surgery he needs will be fully paid for once he signs up.

Yes, moving isn't free. Do you have any relatives or good friends in such a state who will help?
