I don't know what's "standard", but the LBS I bought a $1500 bike from earlier this week did a like for like stem swap for free. They had to order a new $40 stem, and they kept the original stem. They said it was part of the fit service. I noticed when I was in the shop picking up the bike, they had a box of take-off stems they were selling for $5, so I don't think I was special in any way.
They also installed a bunch of stuff I already owned (pedals, bottle cages, etc) for free.
A $40 stem the shop bought wholesale for $20 and might turn around and sell the trade-in for something in the range of $5-40... so, they took a perhaps a $15 hit to make a customer happy but, what about the bike. What was the full retail price. Bikes I've bought are usually $300-400 off that price-- sometimes a lot more than that.
