Pretty ironic that in your own words every bike you ever bought needed a stem swap yet your wife just bought one and didn’t need a swap as you would have been aware of the shop’s practice had she needed one.

Btw, how long after the initial sale should the LBS be bound to swap out the stem at no charge?
Same day only? A week, a month?

I expect bike sizing is based on some presumed average body dimensions. If your dimensions aren't average most off the shelf bikes probably need adjusting.
Ever buy a pair of pants that needed hemming? Did you feel guilty because the next person could wear them out of the store.

This is not a questionable practice. The bike shop buys products its customers need and then re-sells them at a profit. That is their business model. You needed a bike, but because of your physical dimensions, you also needed a non-stock stem. The bike shop paid for the bike and sold you the bike at a fair price. The bike shop paid for the stem, so they sold you the stem at a fair price. Why do they owe you free stuff?

If you go to the grocery store and buy a nacho plate, but the nacho plate doesn't come with guacamole and you *really* like guacamole on your nacho plate, do you try to talk the checker into giving you free avocados, since that's the way you like nachos? Of course you don't, you don't want to be that guy. So don't be that guy at the bike shop either.

If it was the same type of stem from the same company then I'd expect an even swap. I've never paid for a stem change. For example, on the Roubaix I bought last year, as long as I chose a similar quality stem and not a carbon stem or upgraded stem I was free to swap until I found one that fit me correctly. I spent a total of about 3 hours on a trainer at the shop getting it dialed in.

I did have to pay for a saddle swap because I'm a big guy and it required a significantly wider saddle than was available in the standard saddle designs.

If you upgraded stems and didn't just change the length I would expect them to charge unless you worked a deal in advance.
