Ghost pedaling is one sign of too tight an axle bearing adjustment. I set the AWs I service to have a very slight bearing slop if felt at the rim. Another nono is too tight a chain. What cranks does the bike have? I would check both the BB condition and adjustment as well as the crank arm retention Cotters can loosen and/or get worn on the wedging surface and cotterless can have arms loosen on the BB axle.
