It sounds like your RD hanger may be bent. You can make an alignment tool from an old axle and a paint stirrer stick, or use a wheel with an axle that protrudes far enough to screw into hanger, and also provides plenty of leverage to bend the hanger until the two wheel rims are parallel. Don't bend the hanger any more than you need to; they will break if pried back and forth too much.
