Makes me wonder if a PO thought it could be removed, went at it with a hex key, and caused the damage we see here.
Anybody else have a close-up of a LeMond with this same version of bottle bosses? Because I'm truly curious.

Might be a part of the insert tool which holds it in place is hexed. The threaded bit that draws it tight would pass through the hex head of the pliers.
