Ever since I grew into my feet at 16y.o. I discovered that getting the next normal (11 D) size up got me the extra width I need (10 EEE). The extra length no longer bothers me at all. I've been doing this for both dress and sport shoes for decades. I sometimes get cycling shoes two European sizes longer which pretty much does the same thing. WAY easier to find just about any (un)reasonable length you can imagine. Finding shoes even a little bit wider than normal, requires witchcraft at best, and outright necromancy at worst.
