You can buy the extenders at the Serotta web site,
www.serotta.com. Look in their store, under parts or accessories. They press into the headtube, and then you install the headset as you would normally. They come in 1" and 1-1/8" versions. Just remember, the extender only works if the steerer tube on your fork is long enough to accommodate it.
