How tight do you torque it? The recommended installation torque is about 35 foot-pounds which is unbelievably tight. If you don't have a torque wrench, you will just about never tighten it adequately. A normal length hex wrench is totally insufficient.

I am a little unclear. Is it the bottom bracket that is coming loose or the bolt that secures the crank to the spindle? If it the second, you can use a hex wrench and a small piece of pipe as a cheater bar. The added leverage will help you get it tight.
