Exploding inner tubes
Recently bought a pair of Conti Mountain Kings 26x2.2 to stick on the wife's mtb. I've blown around 4 inner tubes, at the valve. The tubes have all 26x1.175/2.125, both kenda and CST. I tried mounting them on a different rim with the same results. I had them go at as little as 40psi. Max psi on the tire is the usual MTB 65psi.
I've been changing tire on bikes for years and can count the number of times I've had tube blow outs on one hand and that was due to over-inflation. I'm fairly sure I'm not getting the tube twisted in anyway and the gauge on the floor pump, seems ok. One tube went when a still quite soft to the grip.

Sounds like a pinched tube to me, also. After I get one side of the tire over the rim, I partially inflate tube, rub it in baby powder, & install. Make sure valve is aligned & straight. Then deflate tube just enough to get other side of tire over the rim. I never use tire levers. Slowly inflate , using a BIKE pump ,a little at a time, while checking to see that the tire is seated evenly all the way around BOTH sides .
