rear axle problem

rear axle problem
Hi guys, i was wondering if someone may be able to help me diagnose a problem with my bike and tell me if its self fixable or I need to take it to someone to fix.
I have been getting a creaking sound coming from the peddles when i press down on them for a few weeks, and had assumed I just needed to oil somewhere on the crank.
However when I went to cycle home on Friday night the chain kept slipping off the rear smallest cog. When I finally got the bike home I found that what im assuming is the rear axle on the bike was not connected and the rear forks could be pulled apart. I checked all the nuts and none of them were loose. Do I need a new rear wheel or what?
If anything else needs answering I will do my best....

Well... you'll want to remove your rear wheel and see what is going on. But off the top of my head I'd say it sounds like you have a broken axle.
