Sugino crank puller
Are these Sugino crank pullers rare? A buddy of mine has two and they work great for the nutted bb spindles on a couple of my bikes. But, they seem hard to come by. I notice only one on Ebay right now.

I don't think they are rare. Park still makes them and I think they are under $20. They work for just about all square taper cranksets. Mine is a Shimano more than 30 years old and it has happily extracted Campagnolo, SR, Shimano and Sugino cranks as far as I remember.

I have the Park Crank Extractor (Crank Yanker?). I really like this one from Nashbar, which only cost me about $8 on sale. I guess if somebody calls you and tells you these are rare, they are probably crank yankers.

I've got similar one from Performance. Works great on the bb spindles that take a bolt because the end of the spindle sits further inside the crank opening allowing the puller to thread on. However, on the bikes I have where the spindle takes a nut it won't thread on because the end of the spindle sticks out further. The Sugino one threads past the extractor that rests against the spindle allowing it to thread onto the crank. I went ahead and pulled the trigger on the one on Ebay. Not sure if it was a fair price or not but I want to make sure my friend doesn't get irritated with me always borrowing his crank puller.

CBO sells several versions of the Ice Toolz puller which is similar to the Sugino. The deluxe version sells for $13.49 and I have used it for quite a while and like the way it works. My old one was branded Lifu but they are the same. The new version has a magnetic tipped adapter for both solid and hollow spindles. It also includes a 14/15mm crank bolt socket and 8mm allen tip.

I have a Sugino crank extractor tool, but doesn't resemble the one pictured. Mine has a rotating insert at the pin end. I believe the one in the OP's pic is for nutted spindles. As mine only fits spindles for bolts. I don't believe these are so rare that you can't find alternatives still. I prefer my Wrench Force by Snap-On crank extractor. Similar to the Sugino but removes cranks from both spindle types. Costed me $4.95.

I bought one of those Sugino pullers back in 1973 when I had my Nishiki International. It had Sugino Maxy cranks.

No. The thread on the Sugino tool is also 22.0mm x 1.0mm. The only significant difference is the domed end to push on the nutted spindle and a 14mm (rather than 15mm) socket to remove the nuts from the spindle.
