Put some sugar in to her gas tank.

If you're going to deal with this on your own, starting a police record with your name on it and her info will link you to any mischief that might occur.
If mischief isn't going to occur, talk to the cops. I've never known anyone who acted like this who was approachable by strangers, much less anyone who might come knocking on the door with a complaint. $5 says she'd call the police and tell them you tried to kick in the door while shouting bloody murder and waving an axe. Not a good way to get aquainted with the local legal assistance when you might need them. If you can convince an officer to make a visit, however, she'll probably give you (and other cyclists in your area) some space next time while muttering unspeakables about stupid police not having anything better to do than harrass her, she oughta sue those ******** penniless.

Leave an anonymous note at her residence. (on her car or in her mailbox) This can be done without letting on who the note was from.

Inform her that while motoring you saw what she did to that cyclist and did not approve. Adding a vague insinuation of possible retaliation can be helpful. Since she now knows that someone watching her knows where she lives, this will most certainly scare her straight better than anything else. No one would be comfortable in the letter receivers position.

When you see her pay her no special attention of any kind. Do not wave, do not go out of your way to ride by her house, do not approach her. (you didn't write the letter) If she wants to go to the police with the note (unlikely, as it would also incriminate her) it can not be pinned on you.

Call the cops and report her. If it happened once or twice I might let it go but she is doing this over and over. She is endangering your life and she needs a wakeup call. Hopefully, the cops will pay her a visit and scare the crap out of her dumba$$.

This is always my first line of "defense", and you were correct in your OP that typically when you take away their sense of anonymity the problem will typically go away.

One way I will make people who I am VERY suspicious of selling or buying drugs uncomfortable is to make a very big deal of stopping right in front of their car in a parking lot and making a very noticeable production out of writing down their license number. If the car is backed in, and I have to get out and walk around the car even better. Some of the more aggressive dealers will ask what you are doing, and I reply with a smile, "O, nothing" and drive away. I could see this type of ploy working for aggressive drivers as long as you did not actually confront the driver.

p.s. Just offering another solution here, but I would also recommend calling the cops when this type of thing happens. Especially if you have a plate #. This is the ultimate form of stripping the anonymity of the offender.

Since I waited too long to take action, I think that I really need to wait until 'next time' to report her. However, I have decided to wave (5 fingers, open palm) every time I see her in the future. Part of it is to let her know I know who she is, but also people seem to be programmed to have a harder time screwing with someone who acts friendly. I may get a helmet cam.

After my ride home last night, I decided I needed to chill out a bit. I always use my mirrors, but I've been scanning more than usual looking for her. Well, last night, I see lights from a truck that looked like hers coming up accompanied by a little right drift, and then a honk. The honk was a bit tentative, and just as I turned up my head to burn as many details as I could into my mind so I could call the cops immediately with a great description, I realized it was a friend.

My friends honk at me because I tell them to (and they know to do this ONLY to me). Keeping my reflexes trained helps me stay calm and decisive when the real thing happens. For the time being, I'll quit hunting for her and just wave when I see her. We'll see if it makes a difference.

+1. You really want to get some kind of paper trail started on this, so that if the byotch ever hits you, the DA has evidence of a pattern of reckless, if not felonious conduct.
