Rear Thru axle loosening up---WTF?
Yeah I know its not the end of the world to have to tighten your thru axle every 50 miles or so, but once I forgot it cocked the wheel and unscrewed the nut holding my cassette on the wheel...so I check it and tighten it every ride if needed. Do all you mountain bike folks do this? Ive tried using my nut driver to tighten it via the nut/screw on the axel, but it keeps loosening up? Anyone got a fix for it?

Exactly what do you mean by "cocked the wheel"?
I'd check the surfaces of what'd be the locknuts on a regular wheel. See if there's any "bite" in those.
That configuration, you have brake forces pushing the axle back, then you have the drive forces pulling it forward. If there's any slop in the axle assembly, and if the t/a isn't closed firmly enough, the wheel will work itself loose as soon as it starts sliding.

It sounds like the skewer came all the way out of the threaded side. Maybe he needs one of the wrenched skewers, unfortunately the threads are not standard between brands. See, for example, these: SK4082: PMW Rear Combo Head, 157 mm x M12 x 1.5

What I mean is...the first time it happened the axle came all the way out and it unscrewed the lock-nut keeping the cassette on the wheel. Turned into a mess that I had to repair on the spot. I check it much more frequently now and it never gets a chance to come all the way out.

Permatex sells a "thread locker" compound that can help. Get the "blue" version. It can be found at any automotive supply store.
