If it's a medical problem, you should see a doctor...

Otherwise, I'm not sure what the problem is... tight pants too tight? oversized scrotum?

Have you tried not wearing underwear under your cycling tights?

Hi, i do not wear any tights, just plain old fruit of the loom cotton brief,a loose cargo pants outside, dont have any medical problem with my scrotum, it's just the bottom part of the scrotum get pinch between the body and the brief against the saddle, the saddle is perfectly leveled,i might try some real tight underwear or a jock strap

Try cycling "liners"- a padded brief made to be worn under regular clothing.

Under Armor makes a good athletic supporter type of boxer brief. They keep the boys in place, are comfortable and wick moisture away.

I was referring to their Heatgear Boxerjock, which comes in varying leg lengths. Here's a link with specs and reviews.
