currently using Lake shoes.
my foot is really narrow, often use two layers of thin socks with them.
so, I'd say they run about medium width.
toe box is pretty roomy, no issues with pinching there.
the BOA closure system is pretty sweet.

The Lake lasts are some of the best I have seen for my feet. I now own four pairs. I am not a fan of straps and have removed them from 2 pairs and installed grommets and laces. (Wow! $10 turns compromises that I had to adjust multiple times a day to riding for 5 hours never thinking about my feet and all I have to do is lace them up with full attention.) I have also found the Lakes, even the cheapest pairs, to be shoes that hold up very well; to the miles, to being ridden wet, even very wet (like fix-gear through 30' long, near BB deep puddles), for years.

I've been using the same pair of Lakes since 2008, I love them I have about a 10.5 US foot and ride a 44 Eu size Lake. The only thing I've done to them is adding little bits of rubber or leather to the toe and heel of the shoes. Mine are full carbon and I started to really wear it down so I started glueing little bits to the toe and heel to make the last. It also makes them much more walkable.

True to size. I wear an 11.5 and have 45.5 Lakes. I have the CX217 Wide and the CX237 Regular. The regular in the CX237 is almost as wide as the CX217 Wide. They are great shoes, especially for the money. I have slightly wide feet so it's hard to find nice loooking shoes with BOA lacing in wide widths that aren't $400. Lakes fit the bill perfectly.

I wear an 11.5 wide or a 12 and 46 normal Lakes are just a bit snug. 46 normal Giros fit fin by comparison ... I'd agree Lakes seem to run a little smaller than marked size.

Just be aware that if you have a fairly narrow heel, you can get some heel-lift with the Lake design.
The width of the front of the standard shoe is great, as others have mentioned.
I would have chosen Lake for my recently purchased shoes (chose a Shimano model instead), if not for the heel-lift.

Yeah I'm a 9.5 wide, and my Lake CX237's are 43 wide and fit great. You might want to look at the page on their website about their lasts, though. Not all their road shoes use the same last, so which model you buy may impact fit somewhat (it seems the higher end shoes run a bit narrower, or at least are meant to fit more snugly).

Just bought a pair of CX237's in 45 Wide for 10.5 US high volume feet. So far, I love these shoes and they got the thumbs up from my fitter last week after looking for well-fitting shoes for three years. I love the dual BOA system and so far they are very comfortable. I added Specialized BG footbeds for a little more support.
