this makes me very nervous about my new (used) cannondale with carbon fork/carbon steerer which I installed myself and is a little bit jiggly!!

Well, that clears up the mystery behind the accident. Clearly, the beer bottle came untied and fell into the front wheel, hitting and severing the fork.
