Well, it sounds like I jumped the gun on Bonk #1 last weekend - but I'm pretty sure I have a lock on crash #1...
Went out for a repeat of the same 47-mile loop I did last week, this time packing some nutrition to avoid hitting the wall. Coming down a nice incline at a good clip, I spotted the gravel in the left turn just a bit too late. Dumb - I had noticed it the previous week, but am not all that familiar with the roads yet. This time, the bike kept going straight, and my left side slammed into the macadam. The helmet I'd bought two days prior did a great job of protecting my head (I'll be taking advantage of Trek's crash warranty) - but my shoulder didn't fare so well. Didn't realize that it was hurt pretty badly until after I rode home 16 miles (didn't feel so good when I had to stand in climbs...). At that point, I just shut down, spent the afternoon in the ER, and came home with a sling, a Rx for percoset, and a diagnosis of third degree A-C separation. I guess it will be a while before I ride again...

Don't blame yourself over it, crashing on gravel isn't anything to be ashamed of.

I had a similar shoulder injury when I was cut off by a car and flipped over the hood. Used my hand to break the fall and tore the ligament. Had a couple screws in there for a while. Now my arms are different lengths.

As for the bike, I seem to have gotten off pretty luckily. I think I took the brunt of the crash personally... Besides some easily adjustable/fixable problems, the main damage is to the right shifter, which was knocked about 30 degrees inward (difficult to see in the pic below due to the angle) and lost its fancy little "Ultegra flightdeck" plastic cover. Shifting isn't as smooth, I hope it's just a matter of readjusting the derailleurs.
