Like I said, either it's much better by Monday or off to the doctor I go. RUQ pain would most likely be gallstones, If Ultrasound negative get a HIDA scan. Ifr these have been done and it is like prior episodes of reflux, then obviously reflux is the likely culprit. If it is not responding to the prilosec, you may have coexisting H. Pylori infection which would require antibiotics in addition to the H2 blockers ( prilosec) Couple of ancillary things. 1) do not eat anything for 5 hours before bedtime. 2) No fatty or greasy food. 3) No alcohol 4) Avoid carbonated beverages. 5) No chocolate or mints. 6) Raise the head of your bed up on 6 inch blocks if the above does not work 7)obviously-avoid spicy food, acidic food like orange juice, tomato products. If you are taking OTC prilosec, your not getting enough. Take 40 mg prilosec in am and in pm. You can also supplement this with antacids like tums as needed. If it persists despite all this you need an upper endoscopy to r/o other causes. Hope this helps, and oh, did I mention ....no sex for six months. |

On a more serious note, pcad, there's something odd about the pain. Pain from an ulcer shouldn't be radiating around to the back or flank. I'd absolutely get an EGD and ultrasound and probably a CT if the ultrasound doesn't nail down a diagnosis. It might be paranoid, but pain going to the back suggests more pancreas/bile duct issues. I mean, it probably is just regular old PUD, but when symptoms are out of the ordinary it's always good to cast a wider net. |
