crashes: rarely to never
last time i fell biking must have been when i was like 12
close calls: occasionally to infrequently
in SF i have had a couple of cagers( read: 3 in the last few years) try to make a gap in traffic on a left turn while on cell phones and come within a few inches/feet of splattering me
in SB i just have to deal with drunken idiots/freshman girls on beach cruisers which is more likely to actually make me crash, but less likely to kill me

What "drivers should do" isn't going to make you any less run over when they go ahead and do something else. Even if you're in the right place, you should always be watching for cars to cut into your path, and you should always know when you're in a spot they're likely to cut across. A driveway into/out of a busy plaza is a place you should be prepared to get cut off regardless of whether the driver *should* yield right of way or not. *should* means absolutely nothing, we as cyclists need to deal only with *is* *does* or *did*.

If you take the same route you'll get used to the traffic patterns, etc. I only have close calls when I'm in a rush, usually.

Rarely, but I had a really neat one today.
While riding thru a downtown area, with very light traffic. I was in the right hand lane of a 4 lane street, about 4' off the car doors. A young (considering my 62) woman came roaring up behind me, layed on the horn, and yelled something profane at me as she passed. I almost yelled back, but she went by too quickly.
The really cool part, was that she failed to recognize the bicycle cop next to her in the other lane, waiting to turn left....
He flipped on his go fast lights, and siren, (color me surprised!)and pulled her over about a block down the street, when she caught the next red - and then stopped me too as I rode by. He just wanted my name and phone number as a witness, to add to her summons for aggressive driving.....
We do have a fair sized contingent of bicycle cops in the town where I frequently ride, see them on the trails, parks, and downtown areas frequently. They always wave, and exchange pleasantries.
That had to be the coolest thing I ever saw happen!!!!!!!
I sure hope I get to go to court!!!!!!!

lets see i ride to my local gym in my town, little hole in the wall place. and when i ride home (which is only 1/4 mile if that. its usually at least two close calls a day. people backing up out of parking spots, people turning into parking spots with no blinkers and or not looking. i've even had to ride over the yellow lines so this one lady didnt hit me pulling out of a parking spot.
