heres mine, just built it , 77 peugeot, Pake 46 tooth crank, 16 tooth cog with 1/8 RED chain BB is a shimano 68mm with english threads.Dont know why, but it fit perfectly, threaded opposite instead of same direction.

Use blue lok-tite on the cup threads and the problem will disappear.

You have described cups backing out.
