This comes up occasionally at the bike coop, so I finally
sent away for a pair of the #3 Jacobs Chuck wedges
that have shown up from time to time as the solution.
They worked well when most recently a guy brought in
his bike with the crank he'd stripped by accident in attempting
removal. My guess is they will work for you, too.

Bikes: too many bikes from 1967 10s (5x2)Frejus to a Sumitomo Ti/Chorus aluminum 10s (10x2), plus one non-susp mtn bike I use as my commuter
I've been suggesting these for over 30 years, and sold a decent number back when I was a regional distributor. I'm almost certain that the #6 wedges are will work on Isis or other OS spindles. If not it's so close that you can easily widen the slot by passing it over the side of a grinding wheel.
