Wierd ankle problem...

Wierd ankle problem...
So I've got an odd one going on here...
It started last fall when I was riding my Time RXS pedals and using cleat wedges to cant my feet out. I decided to play around and remove the wedges to see if I still needed them. At first everything was ok, but after a few weeks things went bad. I started getting wicked inner ankle pain and I started noticing my arch on the left foot was dropping. I went and saw a podiatrist and he checked things out and said 'posterior tibial tendonitis'.
I put the wedges back in, scaled back the riding (it was off season) and things improved.
Now that spring is here and the training has ramped up the inner ankle issues have returned (no where near as bad as before), but it seems they only tend to happen when I'm seated and pushing the cranks...ie. climbing or really pushing the watts...If I stand things tend to be ok...
For example, today I did a ride that involved 50 km of warm up easier pace riding that was taxing my ankle a bit, followed by 5 repeats of a local 350ft climb. I always tend to climb standing more then seated. I was worried that the climbs would do me in, but by the second time up the ankle pain was gone. I completed the repeats with nary a complaint for it. Now I sit at home and it feels the best it's felt in few days..
