Trying to pin down the maker? No idea. The cycling world was smoking a lot of crack in the 80's and 90's. Coincidentally, team USA was smoking a little bit of that crack at the Olympics this year too. Maybe try C&V?

Wheels will definitely be staying with the frame.
Heard from the builder that he remembered building quite a few frames with the narrow fork, but unfortunately he didn't recall where the hubs came from...so there must be some other examples out there somewhere.

It looks like Moulton makes, or at one point made, a 70mm O.L.D. front hub. This website claims to sell them, though I cannot vouch for the website. I also found an auction on eBay Ireland for a Hope Mini front hub, but I couldn't find any reference to such a thing on the Hope website. Sheldon Brown's website also suggests that there were a couple folding bike makers using 70mm O.L.D. front hubs. If you can find any of those, you can probably fudge the 1mm difference.
That's all I have for you, though.
