Are you supposed to tighten it when it is shifted to the big ring or the little ring?

  • shift to the littlest ring
  • adjust the barrel adjuster so you have some room in both directions
  • put the cable on - side of the bolt furthest from the pivot (probably closest to the frame)
  • pull the slack out
  • tighten the fixing bolt
  • adjust using the barrel adjuster until shifting is buttery smooth.

One thing I'm still not clear on is whether the cable is supposed to be in direct contact with the bolt or if the little washer is supposed to be on the outside instead.

Washer. Always. The bolt would chew up the cable.
