As an OFA/EMR, as someone who worked at an ABI/SCI (acquired brain injury/spinal cord injury) rehab and as someone who's mother-in-law died from an otherwise avoidable head injury I'm all about the helmets. $20 or $200, as long as it meets all the requirements then bravo.
People just don't consider the long term costs associated with head injuries whether it's the cost on the health care system or the burden put upon family members.

Brain injuries are forever. There's no coming back. Not to where you were before.

Last accident had me hitting the head hard on the pavement. I've decided since that the inconvenience of a helmet is worth it.

I have two, a summer helmet, and a winter helmet. The winter helmet is a knock off copy of the summer helmet.

Summer helmet:
vintage Bell Tourlite
$10 Ebay
not until/unless I have to, then only if when I can find another one.

Winter helmet:
OGK knock of of a Bell Tourlite, with the vents taped
$5 at a yard sale
same as above

If I destroy one or the other, then which ever one is still around will wind up being worn year round, as opposed to being a summer or winter helmet.

I think the Bell Tourlite is/was the best looking helmet. The ones they make now just don't do it for me.

I just replace my old Bell with a Louis Garneau Orbit helmet as a specific commuter helmet with all my attacthment goodies. The headband adjusting knob works well. The vent is position right for my helmet flashlight, Contour HD and PBSF......there's even room left for my head

Don't recall what was full price, but I paid only $4 after my points from Bikeperformance.

Hmm.. If I recall correctly it was ~ $80 (sign of a good purchase)

This was a helmet that replaced a totaled helmet in a MTB-ing accident that landed me in hospital with concussion.. My helmet, no doubt, saved my life... To answer the question: When it is ~3 years old, or showing signs of wear, or takes a serious blow... whatever comes first...

and no.. I do not ride without a helmet.. My brain is my number one work tool.. no way on the planet I want it traveling at 20 miles an hour, a meter away from a stationary, hard, totally immovable object (called the ground)

Trek It's blue with lot's of vents and a visor.

2) What did you pay for it?
Got it with the bike off CL... $200 IIRC

3) And when/what will make you decide to replace it?
When it saves my noggin, get's lost or STOLEN! :-)

Some sort of Bell, I think.
$10 at K-Mart, if my memory isn't playing tricks on me.
I was in college when I bought it, which means it can't be more than 15 years old, is definitely more than ten years old, and won't be replaced until I fall on my head. That's happened three times, and the time when I was wearing a helmet (the second), while not pleasant, was definitely the least unpleasant.

2. Around $100 US.
3. When the pads are all destroyed it becomes my winter/light mounted helmet and I buy a new one.

I only want one helmet for riding, so I go with the more expensive helmets that are lighter with more vents so it works for road riding and commuting, but I typically get the mtn bike versions so they have a visor which keeps the rain off my glasses.

2) ~$30, it's been like 6 years since I bought it so I'm not sure of the exact price. Bought it at Dicks Sporting Goods.
3) I'm not pleased with its ventilation properties, and it's dark grey so now that I'm commuting more ideally I would like to get something more visible. I'm thinking of getting a higher-end helmet. Looked at the Bell Volt at my LBS, and it fit great... but the price doesn't seem like it will fit my wallet very well ($175) . Considering a Bell Array at the moment ($100). Might upgrade to Bell Sweep for the removable sun visor ($140).

1 - Bell Citi. I like the tail light mount, the color, and the reflective accents. And really like the (sold separately) mirror made just for its visor. Oh, and it's pretty comfortable too.
2 - ~$30, on clearance at REI
3 - If I deem it no longer to be safe. If that were right now, I would probably replace it with a Bell Muni. The Muni has integrated rear light, accommodates the Blackburn Flea front blinkie via a 'perch' on the visor, and also will mount the Bell Flip Mirror.

I liked the idea of the Bell Citi (rounded back, mounting point for front and rear lights, available in bright yellow, compatible with flip-down rear-view mirror), but I never found it comfortable, and I thought it needed better ventilation. Then Bell released the:

1. Bell Muni. It has all the good features of the Bell Citi, but fits way better, has better ventilation, AND now it has a pair of red LED blinkies in the rear straps built-in. The optional flip-down rear-view mirror is a godsend; great view to the rear when down, and out of the way when flipped back up into the visor.

2. $70 for the helmet and another $12 for the mirror.

3. I'll probably replace it in 5 to 7 years with the exact same model if it is still around. Or if it saves my noodle in a crash.

My last helmet was a 25 year old Vetta Corsa, which I held on to for way too long. I really love this new one. Highly recommended.

2. less than 30, can't remember exactly..but I'm a grad student so I wouldnt have been able to afford anything more that that,
3. I think I'll borrow sci_femme's great idea in 4 years. and then buy a rounded helmet, like the nutcase. I feel like mine makes me look like sonic hedgehog

My head fell on the curb during a fall when I was in 7th grade. Thank God I was wearing a helmet and I suffered no injuries. And now, I always ride with one.

but let me ask you the people you see coming in, do they have head injuries mostly from cycling, or is cycling rather uncommon among all head injuries from your experience.
