I wear jeans and sneakers and no lab coat. Sometimes I wear a warm-up jacket. I am uber-casual.
I spent a lot of time crawling around on the floor with my patients.
Sometimes I call them "Sweetie" or "Sweets" or "Sweetie Pie". I also do a lot of sweet-talking and cajoling.
I draw lots of pictures for people about how their dog's wiring is laid out and why the symptoms mean the problem is specifically in X location in the nervous system. But I never have a pen (because I have no lab coat pocket to put it in), so I always am bumming pens off people and then immediately losing them.

Okay, so my schedule is set for 30-40 patients on a busy day. This figures a certain percentage of quick followups, a certain percentage of new patients that require more time and space for add ons that are emergent.
Everything is going smooth until patient A comes in with follow up studies that reveal a cancer that was not suspected. The 5 minute followup slot is now 45 minutes with a crying patient. In the middle of this the on call doc calls me into surgery to help out with a disaster. The point is as much as we try to respect others time and as much as we try to adhere to a schedule that is realistic, every day is different and every patient wants you to spend time and not rush through their appointment. It's a crap shoot. Sometimes everyone is happy. Sometimes everyone is pissed. But it's all in the service of trying to do the right thing and respect everyones needs.

BTW, everyone deserves days off. I never complain about anyone's hours or how much cash they make and that includes doctors.

BTW, what Stoney was talking about is different than what you experienced. Your reschedule at the last minute was either a scheduling screw up- double or triple booking (which most doctors just suck it up and see anyway). Or last minute procedure that had to be scheduled, something urgent. Or the sudden need for the doc to take the day or part of it off- sick kid, crown that fell off, or appealing tennis match.

I had a 3.5 days/week schedule once. I worked 20 hours at night, 20 hours during the day at the MSU library, they let me combine nights and days so I had 13-15 hour days.
Long days of sitting in front of computer doing nothing, but having every Thur-Sunday off fresh out of college while still living in a college town was some of the best years of my life.

Honestly I don't like casual Friday. I feel obliged to wear jeans and I almost never wear jeans otherwise. Dress pants are more comfortable. Then I have to go out and buy some stupid business casual shirts that look okay with jeans. So, now my closet/dresser has my regular work clothes, my weekend clothes, and crap I wear to work on Friday.
